Welcome to my site!
I am the school nurse at Norco Elementary. I am here on the Norco school campus on Monday, Wednesday, and Friday from 8:40 am to 3:50 pm. I have attached various medical forms that may be necessary to complete or update during the school year. Please contact me by phone or email if you should have any questions. Stay happy and healthy!
- MEDICATIONS: Students with medical diagnoses that require medication during the school day will need a medication order form completed by their doctor. This may include rescue inhalers, Epi-Pens, and ADHD medications. A parent/guardian must sign the medication in with the School Nurse. NO STUDENT CAN CARRY ANY MEDICATION TO SCHOOL WITH THEM. This includes topical creams or ointments, any oral medication, antibiotics, inhalers, cough drops, medicated lip balm, nose spray, eye drops, etc. Also, medication paperwork has to be new every year dated on or after July 1st; it does NOT carry over from year to year. - SPECIAL DIETS: Students with food allergies or dietary restrictions require a Diet Order Form to be completed by their doctor. This is a form that is required by Child Nutrition in order for them to flag the student’s account in the lunch line and assure they don’t get foods or drinks that they are allergic or sensitive to. A new form is required every school year dated on or after July 1st. Please stop by the office to pick up an order form or use the link below to access my school webpage for a link to the form.
- IMMUNIZATIONS: Students are required to be up to date on state-mandated vaccinations. There are immunizations required at ages 4 years, 11 years, and 16 years. Also, two doses of Hepatitis A vaccine will be a new requirement for school entry for the 2023-2024 school year. Please check to make sure their shots are up to date and make an appointment ASAP with their doctor to get caught up on past due immunizations. Please submit a copy of the updated immunization certificate to the school. Please see my school webpage for a link to the LDOE Immunization Exemption Form if needed.
